- Employer Identification Number, also known as EIN

- National Provider Identifier, also known as NPI
- Medical license information
- Practice or institutional affiliation
- State or states of licensure
This information is collected solely for account eligibility verification and regulatory compliance.
2.3 Account and Transaction Information
- Account login credentials, stored in hashed or encrypted form
- Order history, invoices, credits, and fulfillment records
- Payment transaction references processed by third-party payment providers
- Shipping confirmations and carrier metadata
- Cart activity, checkout activity, and abandoned-cart status when applicable
NeXera Bio does not store full payment card numbers or banking credentials.

2.5 SMS and Mobile Messaging Information

3. Explicit Prohibition on PHI
NeXera Bio does not collect, store, or process Protected Health Information, also known as PHI.
Users are strictly prohibited from submitting PHI, including but not limited to:

- Diagnoses or treatment notes
- Lab results
- Patient images
This prohibition applies to all order notes, uploads, support tickets, emails, SMS messages, mobile messages, and portal communications. Any PHI submitted in violation of this policy may be deleted without notice.
NeXera Bio is not a HIPAA Covered Entity and is not a Business Associate.

NeXera Bio uses cookies, plugins, pixels, and similar technologies for essential website functionality, secure account access, performance monitoring, fraud prevention, analytics, cart functionality, checkout functionality, and abandoned-cart tracking.
NeXera Bio’s website uses cookies and related e-commerce or messaging plugins to help keep track of items you place into your shopping cart, including when you have abandoned your cart. This information may be used to determine when to send cart reminder messages by email, SMS, or mobile message, where permitted and where the recipient has provided the required consent.
Browser settings may be used to manage cookies. Disabling cookies may affect website functionality, login access, cart performance, checkout functionality, and account-related features.
10. Location Data
NeXera Bio does not use precise GPS location tracking or location-based mobile services. We may collect billing addresses, shipping addresses, and approximate location information derived from IP address or device data for fraud prevention, security, tax, fulfillment, logistics, and account verification purposes.
If NeXera Bio later offers any feature that uses precise location tracking or location-based services, this Privacy Policy will be updated to describe how that information is collected, used, and disclosed before such feature is implemented.

NeXera Bio operates exclusively within the United States. Information is processed and stored in the United States.

The NeXera Bio mobile message service (the “Service”) is operated by NeXera Bio (“NeXera Bio,” “we,” or “us”). Your use of the Service constitutes your agreement to these terms and conditions (“Mobile Terms”). We may modify or cancel the Service or any of its features without notice. To the extent permitted by applicable law, we may also modify these Mobile Terms at any time, and your continued use of the Service following the effective date of any such changes shall constitute your acceptance of such changes.
By consenting to NeXera Bio’s SMS/text messaging service, you agree to receive recurring SMS/text messages from and on behalf of NeXera Bio through your wireless provider to the mobile number you provided, even if your mobile number is registered on any state or federal Do Not Call list. Text messages may be sent using an automatic telephone dialing system or other technology. Service-related messages may include updates, alerts, and information, including order updates and account alerts. Promotional messages may include promotions, specials, other marketing offers, and cart reminders.
You understand that you do not have to sign up for this program in order to make any purchases, and your consent is not a condition of any purchase with NeXera Bio. Your participation in this program is completely voluntary.
We do not charge for the Service, but you are responsible for all charges and fees associated with text messaging imposed by your wireless provider. Message frequency varies. Message and data rates may apply. Check your mobile plan and contact your wireless provider for details. You are solely responsible for all charges related to SMS/text messages, including charges from your wireless provider.
You may opt out of the Service at any time. Text the single keyword command STOP to +18882775113 or click the unsubscribe link, where available, in any text message to cancel. You will receive a one-time opt-out confirmation text message. No further messages will be sent to your mobile device unless initiated by you or otherwise permitted by applicable law. If you have subscribed to other NeXera Bio mobile message programs and wish to cancel, except where applicable law requires otherwise, you will need to opt out separately from those programs by following the instructions provided in their respective mobile terms.

We may change any short code or telephone number we use to operate the Service at any time and will notify you of these changes. The wireless carriers supported by the Service are not liable for delayed or undelivered messages. You agree to provide us with a valid mobile number. If you get a new mobile number, you will need to sign up for the program with your new number.
To the extent permitted by applicable law, you agree that we will not be liable for failed, delayed, or misdirected delivery of any information sent through the Service, any errors in such information, and/or any action you may or may not take in reliance on the information or Service.
